On a warm, sunny afternoon in April, Mr. Yang burst from his home in this rural village near the Vietnamese border, carrying a kitchen cleaver. He encountered three youngsters headed home from school on the dirt path outside. He hacked two primary schoolers, badly wounding both, and slit a second grader’s throat, leaving him dying on the ground. Then he moved on. By the time police officers caught up and subdued him, he had slashed two more people to death.
The victims’ families have focused their rage on the police. Three days earlier, Mr. Yang had struck a neighbor in the head with an ax, but was not detained.
“They are completely responsible for this,” said Wu Huanglong, the second grader’s father. “They did not protect us.”
But Mr. Yang’s doctors see a bigger failing. Despite clear signs of schizophrenia, Mr. Yang had received medical care for just one month in the previous five years.
“If he had been given medication and treatment, his illness would not have developed,” said Chen Guoqiang, the psychiatric hospital’s chief doctor. “If he had been able to control his hallucinations, he would not have killed anyone.”
It has been nearly 35 years since the end of the Cultural Revolution, when mental illness was declared a bourgeois self-delusion and the sick were treated with readings from Chairman Mao. Psychiatric treatment has returned. But mental health remains a medical backwater, desperately short of financing, practitioners and esteem.
Too often, the official response to mental illness is to look the other way. The government authorities, already shaken by an attack the previous month in which eight schoolchildren were stabbed to death, threw a news blackout over the Xizhen incident lest it inspire copycats or incite further outrage.
At least three of six men whose attacks near schoolyards this year left 21 people dead had earlier appeared deranged or suicidal, according to news reports. But in the highest-level statement on the killings, Prime Minister Wen Jiabao said only that China needed to resolve “social tensions” underlying the attacks.
Yan Jun, director of the mental health division of the Ministry of Health, refused repeated requests for an interview. The ministry said in a written statement that the government was “continuously strengthening” both its resources and professionals to provide mental health care.
A Dearth of Care
It has far to go. Only 1 in 12 Chinese needing psychiatric care ever sees a professional, according to a study last year in The Lancet, a British medical journal. China has no national mental health law, little insurance coverage for psychiatric care, almost no care in rural communities, too few inpatient beds, too few professionals and a weak government mental health bureaucracy, Chinese experts in the field say.
The Health Ministry’s own mental health bureau, established four years ago, consists of three people. Dr. Yan, the director, is a public health specialist, not a psychiatrist.
Every few years, China’s news media declare that a national mental health law is speeding toward adoption. The first draft was written half a century ago. Asked how many revisions it has undergone, Dr. Ma Hong of the Peking University Institute of Mental Health said, “Countless.”
Most psychiatric hospitals are financially unviable, said Yu Xin, who directs the Peking University Institute of Mental Health. One, in Hubei Province, opened a box factory in the 1990s to stay afloat. The fee structure is so absurd, he said, that hospitals can charge patients more for computer-generated diagnoses based on filled-out forms than for sessions with actual psychiatrists.
The Lancet study estimated that roughly 173 million Chinese suffer from a mental disorder. Despite government efforts to expand insurance coverage, a senior Health Ministry official said last June that in recent years, only 45,000 people had been covered for free outpatient treatment and only 7,000 for free inpatient care because they were either dangerous to society or too impoverished to pay.
The dearth of care is most evident when it comes to individuals who commit violent crimes. For example, after Liu Yalin killed and dismembered an elderly couple cutting firewood in a Guangdong Province forest, he was judged to be schizophrenic and released to his brother. Unable to afford treatment, the brother flew Mr. Liu to the island province of Hainan, in the South China Sea, and abandoned him, a Chinese nongovernment organization, Shenzhen Hengping, said in a recent report.
Last year, Mr. Liu killed and dismembered an 8-year-old Hainan girl.
“The government doesn’t want to cough up the money to treat these people, so they just give them back to their families,” said Huang Xuetao, a mental health lawyer and one of the authors of the report.

Then began a problem: what do you do when you cannot go home and you are radioactive?
There are about 40,000 new cases of thyroid cancer a year, and most patients are treated with radiation, which makes them potentially dangerous to people around them for up to a week.
The question of where they should spend that time is drawing new concern from doctors, public health officials and regulators.
In 1997, the Nuclear Regulatory Commission dropped a requirement that such patients be quarantined in the hospital. Instead, patients can be released right after their treatment, when they are at their most radioactive.
And while most people go right home, one patient in New York boarded a bus for Atlantic City, N.J., and set off a radiation alarm in the Lincoln Tunnel. And about 5 percent of patients do what Ms. Maddox did: check into a hotel.
“There weren’t many choices, really,” said Ms. Maddox, who is 72. The night before her treatment, in 2003, she stayed with her daughter in Delaware, but her daughter was pregnant, and fetuses and children are especially vulnerable to radiation. Johns Hopkins had no place for her to spend the night, she said. Getting on an airplane was out of the question because of the dose of radiation she would give other passengers. And there was concern about what would have happened if, as many patients do, she vomited the radioactive contents of her stomach.
Instead, her husband, Bryce, booked a room at a hotel in Newark, Del. “I pretty much went in the back door,” Ms. Maddox said. When they hit the road the next day, heading home, she sat as far as possible from her husband, in the third row of seats in their Honda Odyssey minivan. “I’m sure we looked like we’d had some kind of spat,” she said.
Fortunately, like most thyroid cancer patients, she needed only a single dose of radiation to get rid of the cancer.
Scientists have estimated that, depending on the amount of radioactive drug given to a thyroid patient, a secondhand dose could exceed an average American’s annual level from all natural sources, and three or four times the safe level recommended for a pregnant woman.
One person alarmed about the situation is Representative Edward J. Markey, Democrat of Massachusetts, whose office has been studying the issue. He accuses the Nuclear Regulatory Commission of turning a blind eye to the problem.
“My investigation has led me to conclude that the levels of unintentional radiation received by members of the public who have been exposed to patients that have received ‘drive through’ radiation treatments may well exceed international safe levels established for pregnant women and children,” Mr. Markey said in a statement.
The Nuclear Regulatory Commission took up the issue at a meeting last Wednesday. James G. Luehman, a staff official who helps monitor the safe handling of radioactive materials used in medicine, said his agency might have made a mistake in dropping the quarantine requirement. The idea was that letting people return home to recuperate would cut costs, benefit the patients and allow doctors with no quarantine facilities to perform the treatment.
The agency knew that relatives and strangers would get a radiation dose from such patients, Mr. Luehman said, but it assumed that the doses to people like hotel clerks and chambermaids would be random and that no one was likely to be exposed repeatedly. He recommended that the commission study whether such patients tended to stay at certain hotels near major medical centers, but no conclusion about such a study was reached on Wednesday.
New York City has gone a step further. Last June, it advised radiologists and endocrinologists, “Do not advise patients to go to a hotel.” A few states have similar policies.
There are simple precautions that patients can take to reduce the risks sharply: stay several feet away from other people and keep them from contact with the patient’s saliva, urine and other body fluids.
Radiation from patients decreases both as the iodine dose is flushed out of their bodies and as it loses strength.
Many radiation experts doubt that radioactive thyroid patients represent a public health problem. “We’re talking about really small doses,” said Dr. Henry D. Royal, the associate director of nuclear medicine at the Mallinckrodt Institute of Radiology at Washington University, in St. Louis.

(CNN) -- Federal health authorities on Friday approved Botox injections for the prevention of chronic migraines in adults, an advance experts described as "modest."
In a statement, the Food and Drug Administration recommended Botox be injected approximately every three months around the head and neck to dull future headache symptoms.
The drug -- whose generic name is onabotulinumtoxinA -- has not been shown to work against migraines that occur 14 days or fewer per month, nor has it been shown to work for other forms of headache, said the statement.
Experiencing a migraine on most days "is one of the most disabling forms of headache," said Dr. Russell Katz, of the Division of Neurology Products in the FDA's Center for Drug Evaluation and Research. "This condition can greatly affect family, work, and social life, so it is important to have a variety of effective treatment options available."
Migraines, an intense pulsing or throbbing pain in one area of the head, are often accompanied by nausea, vomiting and sensitivity to light and sound, doctors say. Chronic migraines are defined as those that occur on 15 days or more for more than four hours per day.
The drug, made by Allergan, Inc., of Irvine, California, was approved last month in Britain for the same purpose.
The two company-funded studies submitted to the FDA involved 1,384 adults from 122 study sites in Europe and North America. They found that after six months, patients who got the drug experienced 7.8 and 9.2 fewer days of migraine than they had before the studies started. Those who got injections of placebo (sugar pill) experienced 6.4 and 6.9 fewer headache days.
Over those six months, patients on the drug experienced 107 and 134 fewer hours of headache, versus a reduction of 70 and 95 hours for those on placebo, according to the studies.
"The benefits are modest when you look at the overall results," said Dr. Elizabeth W. Loder, associate professor of neurology at Harvard Medical School and the chief of the division of headaches in the Department of Neurology at the Brigham and Women's Hospital in Boston. Loder was involved in an Allergan-funded trial several years ago but has no current relationship with the company.
"But, of course, within those results, there are always patients who do much better than the average and there are patients who don't have any benefit," she said.
Still, she added, "It will provide more options for patients who currently have very few forms of treatment available to them ... when you are dealing with a problem like this, even modest improvement can mean the difference between being able to go to work and not being able to function very well."
Dr. Joel Saper, director of the Michigan Head Pain and Neurological Institute and clinical professor of neurology at Michigan State University, said he has been using the drug for that purpose for more than a decade, even though it was not approved for it.
The FDA's approval will likely mean that insurance companies will pick up the tab for it now, he said. Saper, who has also participated in Allergan-funded trials, predicted "certain subsets of people will get dramatic results."
The most common adverse reactions reported by chronic migraine patients being treated with Botox were neck pain and headache, the FDA said.
About 1 percent of patients on the drug (versus 0.3 percent of those on placebo) found that their migraines worsened so much they had to be hospitalized, but it was generally well-tolerated, the company said.
In each study, less than 2 percent of patients dropped out of the study due to an adverse event.
The drug labeling warns that the effects of the botulinum toxin may spread beyond where it is injected, causing symptoms that may include life-threatening difficulties swallowing and breathing.
Botox is sold to doctors in 200-unit vials that cost $1,050, said Cyrstal Muilenburg, spokeswoman for Allergan. Last year, the drug had sales of $1.3 billion, she said.
Doctors gave administered a total of 155 units to each migraine patient in 31 injections into muscles of the head and neck, she said. The treatment was repeated at three-month intervals.
Some 3.2 million Americans are estimated to suffer from chronic migraine, though some 80 percent of them have never been diagnosed as having the disease, the company said.
The World Health Organization ranks migraine as the 19th most disabling disease. Women are three times as likely as men to suffer from migraines, which are also linked to depression and anxiety.
The drug is the same chemical entity that was approved 21 years ago for the treatment of strabismus and blepharospasm, two eye-muscle disorders.
Since then, regulators in some 80 countries have approved it for 21 different indications, including treatment of the abnormal head position and neck pain associated with cervical dystonia in adults, symptoms of severe underarm sweatingand treatment of increased muscle stiffness in elbow, wrist and finger muscles in adults with upper-limb spasticity.
Last month, Allergan settled a Justice Department investigation into its marketing practices related to uses of Botox by paying $375 million and pleading guilty to a misdemeanor misbranding charge.
The company also agreed to pay $225 million to resolve civil claims the department was asserting under the civil False Claims Act.
